PROOF OF RESIDENCY
Registering your child for school is a multi-step process that begins with demonstrating proof of residency. HEALTH FORMS AND IMMUNIZATIONS
Parents/Guardians in Abington Township and the Borough of Rockledge who have a child nearing kindergarten age have been mailed a packet with health forms to be completed and submitted with the child’s dates of immunizations. These forms are also accessible via the Immunizations and Forms page under Student Services. Forms should be completed by a family physician and dentist, and submitted at the time of registration, or as soon as possible thereafter.
Pennsylvania school law requires that children must be immunized before original entry into school with the following:

- 4 doses of polio vaccine (4th dose on or after 4th birthday and at least 6 months after previous dose given)**

- 3 doses of Hepatitis B vaccine. The 2nd dose must be given at least one month after the first dose; 3rd dose must be given after 6 months of age, at least 4 months after first dose and at least 2 months after the 2nd dose.
- 2 doses of varicella (chicken pox) vaccine administered at 12 months of age or older or a history of the disease.
* Usually given as DTP or DTaP or if medically advisable, Dt or Td
**A fourth dose is not necessary if the third dose was administered at age 4 years or older and at least 6 months after the previous dose.
These requirements allow for the following exemptions: medical reason, religious belief, or philosophical/strong moral or ethical conviction.
